RE: Dr. Carlston Moving to Kansas
Dear Patients:
As you may already know, I am moving to Kansas at the end of the month. I have not had the opportunity to talk personally with many of you prior to your receiving this letter and I'm sure this is a shock to you. However, please know that this is a decision that I have not taken lightly. Moving a family across the country will not be easy, but it is a necessary step that I need to take.
I had hoped to find a doctor with a specific skill set that I could recommend should you have any future need for chiropractic care, but have not been successful. Please refer to the Washington State Chiropractic Association for a list of competent chiropractors in Issaquah or your respective city. Their phone number is 206-878-6055 and their website is www.chirohealth.org. If you have a clinical emergency prior to finding another physician, please call "911" or go to your nearest emergency room.
Since the records of your case are confidential, I will require your written authorization to release your information to another physician, or to you. For this reason, you will need to complete this form and return it by 8/22/08 to my office. After 8/22/08, please send your request to Healthcare Solutions at 700 NW Gilman Blvd #382, Issaquah, WA 98027. In accordance with Washington State law, your records will be disposed of after 5 years.
Please note, that after 08/22/08, there will no longer be anyone at this number to answer your call. If you need assistance after this date, please seek out a doctor who is a member of the Washington State Chiropractic association who will assist you in finding someone to continue your treatment. Alternatively, you may contact your health care insurers for a list of other providers in your coverage/plan area.
